Peace of Mind Senior Day Center Terms of Service

Welcome to Peace of Mind Senior Day Center (the “Center”). These Terms of Service (“Terms”) govern your use of the Center’s services, facilities, and website. By enrolling a participant in our programs or accessing our services, you agree to these Terms.

1. Services Offered

Peace of Mind Senior Day Center provides social, recreational, and wellness activities tailored to meet the needs of seniors. Services may include:

Supervised activities
Nutritious meals and snacks
Bathroom reminders
Transportation (if applicable)
Health and wellness monitoring
Medication reminders

2. Eligibility Requirements

Be 55 years or older.
Have a completed intake assessment by our team.
Provide all required medical, dietary, and emergency information.
Agree to abide by the rules and policies of the Center.
Diagnosed with Alzheimer’s, dementia, or other memory related issues
Be able to walk on their own with the assistance of cane, staff, or walker

3. Enrollment Process

To enroll in our programs, clients must:

Complete and submit the registration form.
Provide a physician’s clearance if required.
Pay the applicable fees in advance or as outlined in the payment agreement.

4. Fees and Payment

Fees for services are outlined in our Pricing Policy, provided during registration.
Payments are due on the 1^st day of the month, with a 5-day grace period. Anything after 5 days is considered late.
Late payments may incur additional charges as specified in the Pricing Policy.

5. Cancellation and Refunds

Cancellation of enrollment must be provided in writing 30 days in advance.
Refunds for unused services will be issued according to the Refund Policy.

6. Participant Conduct

Participants must treat staff and other attendees with respect.
Aggressive or disruptive behavior may result in dismissal from the program.
Personal belongings are the responsibility of the participant; the Center is not liable for lost or stolen items, but we will do our best to prevent theft of any kind.

7. Health and Safety

The Center follows strict health and safety protocols.
Participants or their caregivers must inform staff of any changes in health conditions.
In case of emergencies, staff will notify the designated emergency contact.

8. Privacy Policy

We are committed to protecting your privacy. Personal information collected during registration will only be used for operational and emergency purposes. For more details, refer to our Privacy Policy.

9. Limitation of Liability

Peace of Mind Senior Day Center will not be held liable for:

Injuries or illnesses resulting from pre-existing medical conditions.
Incidents caused by a participant’s failure to disclose pertinent medical information.
Circumstances beyond the reasonable control of the Center, such as natural disasters.

10. Changes to Terms of Service

We reserve the right to update these Terms at any time. Clients will be notified of significant changes via email or written notice.
